CIRCULAR
It is notified for the information of all concerned that the University has decided
to get fused PVC Identity Cards for teaching/non-teaching staff for treatment under
Direct Payment Facility in the approved hospitals.
This may kindly be notified to all the members of staff (teaching/ non-teaching)
working in your Faculty/Department/Office/College of the University to collect the
application form for the preparation of family medical Identity Card from the Section
Officer, Estab. Branch II (i), Room No.203, New Administrative Block, University of
Delhi, Delhi-110 007.
The family medical Identity Cards of the College staff (teaching/non-teaching)
will be issued and maintained in the respective Colleges/Departments.

The duly filled in forms should be submitted latest by 30 April, 2007 to the
following:

Sir/Madam,
I am enclosing herewith a copy of the circular in connection with the issue of fused
PVC Identity Cards for teaching/non-teaching staff for treatment under Direct Payment
Facility in the approved hospitals.
The relevant points of the circular which are applicable for the Colleges in the circular
are quoted again for ready reference.
a) The family medical Identity Cards of the College Staff (teaching/nonteaching) will be issued and maintained in the respective Colleges.
b) College Staff (both teaching and non-teaching) will submit their forms to
their respective Colleges.
Accordingly the College staff (both teaching & non-teaching) will have to obtain their
application forms from their respective Colleges and submit the filled in ones to their
Colleges, a specimen of which is enclosed.

Instructions:

This Card is issued only for the purpose of taking treatment in the Hospital which are
approved by the University and this card must be produced on demand.
2. The loss of this Card should be reported immediately to the Registrar, University of Delhi
and to the nearest Police Station
3. Misuse of this Card is an offence and will render the concerned University/College
employee liable to disciplinary action.
4. Affix Photograph/Joint Photograph in the space provided for.
5. In case this card is lost or disfigured, a penalty of Rs.100/- shall be
