GHS RENAISSANCE PROGRAM OFF-CAMPUS PERMISSION FORM 2015-2016 _____________ Date __________________________________ __________________ Student Name (Print—Last, First) ID# Dear Parent: Please be informed that your son/daughter has qualified academically for participation in the GHS Renaissance Academic Achievement Off-Campus lunch by earning all A’s (or 85 (B) in Honor’s courses) for this nine weeks. If you agree to allow him/her to accept this incentive, please sign this form below. The following guidelines apply: The Off-Campus lunch must be taken on the designated date each nine weeks: (Your son/daughter must qualify for EACH one): October 30, 2015 January 15, 2016 April 8, 2016 Off-Campus lunch students are to inform their third block teachers of their validated off-campus status, as well as have a permission slip signed by a parent on file and only those students will be permitted to leave. Students will lose this privilege if: a) student is late to 4th block class. b) Is found to be on another school campus during regular school hours.
