Voice Lessons Registration Packet
Voice Lessons Registration Packet Interested in signing up for voice lessons? This packet will get you started. Here are step-by-step directions to reserve your spot with Mrs. Tompkins: 1. Write in your information below. 2. Read the contract with a parent (next two pages). If you both agree to everything, fill it out and sign it. 3. Attach payment of $92.00 to the packet. This covers your registration fee ($20.00) and the first month of lessons ($72.00). You will not receive a lesson spot unless payment is included. Checks should be made payable to Meredith Tompkins. 4. Turn in the packet to your choir directors. 5. Wait to be contacted by phone or email by Mrs. Tompkins! *Please don’t sign up for lessons unless you are willing to commit to an entire semester (or year, preferably). Just like other instruments, lessons are designed to help you improve gradually over time. INFORMATION Name (First/Last):____________________________ Grade:____ Choir Class Period: ____ Lesson Availability Although a majority of students’ lessons are scheduled during choir class, some will be scheduled for before or after school lessons in order to fit everyone in. These are the days Mrs. Tompkins offers lessons: please indicate all available times with a YES. Note: Every year, we end up with limited space. Students who provide before/after school availability are more likely to get spots once Mrs. Tompkins’ schedule fills up. Monday Tuesday Friday Before school 7:20-7:43 Before school 7:45-8:08 After school 3:25-3:48 After school 3:50-4:13 Please list any extracurricular organizations you are in, both inside and outside of school (Ex: Dance, band, soccer, drama). What days/times/class periods do they meet? ______________________________________________________________________ ______________________________________________________________________ ________________________________________________________________ Harpool Middle School Private Voice Contract Thank you for your interest in taking voice! I, Mrs. Tompkins, am very excited to get started with you. My goal is to make my students’ singing experience an encouraging journey. In order to make that possible, I have very clear policies and explanations of how I run the studio and what I expect from students and parents. Please read carefully! If you agree to these terms after reading them, please sign, attach payment, and return this contract to your choir teacher. You will then be contacted to begin lessons! Terms and Conditions LESSON FEES: $18.00 per lesson, due in advance at the beginning of each month. One lesson (1/2 a class period, approx. 23 minutes) will be scheduled per week, with the exception of holidays. A registration fee of $20.00 is charged at the beginning of the year to pay for materials used in lessons and recital costs. I will accept cash, check, or online payment through PayPal Pro or Chase QuickPay. Please note: PayPal requires a 3% additional processing fee per transaction, but credit/debit cards can be used. Chase QuickPay is free, but requires a couple days to get set up with a direct bank account. Checks should be made payable to Meredith Tompkins. Payment is required ON OR BEFORE the due date in order to avoid penalties (see below). Payment is required by the due date because this is a professional courtesy to voice teachers (it is their job). If a family (student or parent) forgets to bring payment by the due date, they will receive TWO exceptions PER SCHOOL YEAR, in which they will be allowed to bring their payment the following week without penalty. After that, the student will not be given a lesson until payment is received. Missed lessons due to forgetting payment will not be made up, but will still be charged as if a lesson was given. Lessons will resume as normal once payment is received. $25 will be charged for all returned checks. GROUP LESSONS: At least once per semester, a student’s lesson will be changed to a group “voice class” lesson, which gives private students in their class period a chance to sing for, learn from, and encourage each other. These will be scheduled in advance and the student will be notified when to be prepared for it. MISSED LESSONS: Please let me know by email, voicemail, or text b eforehand if you can’t make the scheduled lesson, even if it is related to illness . If I am not notified in advance of a missed lesson, there will be no rescheduling of the lesson and it will not be eligible for a makeup or credit. Paying for a lesson is essentially reserving a time slot to work with your voice teacher - that time slot must be paid for even if a student does not show up (without reasonably advanced notice), because the teacher has spent fuel and time in preparation for each lesson. You can expect the same courtesy from me: if I miss a lesson, I will notify you in advance and reschedule a makeup time/day. If a makeup cannot be agreed upon, the student will either be refunded or given a credit towards the next lesson. Tardies: If a student is more than 10 minutes late to a lesson without prior teacher notification, the lesson might not be given, refunded, or rescheduled. CHECK YOUR EMAILS: ALL communications about lessons will be conducted through e-mail (updates, notifications, and monthly invoices). Phone or in-person conferences can be arranged, if requested. After turning in your voice studio registration and first payment, you will receive information on how to access my teaching website. Should you have any questions, comments, or concerns throughout the year, email is the best way to reach me. In an effort to keep Ms. Delaney in the loop, I may copy her in on email conversations. RECITAL ATTENDANCE: If a student misses a performance for any reason, their performance will be rescheduled to an in-class time in front of their choir peers as soon as they are well enough to be back in school. In cases of loss of voice or laryngitis, the performance will be held as soon as singing can be healthily achieved. Student Responsibilities PRACTICE - Practice is actually the most important thing you can do to “get your money’s worth” out of lessons. My purpose is to help you learn how to sing better, to help you develop confidence, and to learn tricks to make your singing stand out. This means that I expect you to learn your music and then bring what you’ve worked on at home to the lesson, so that I can have more time to coach you rather than teach you each and every note. Don’t worry - I will equip you with all the materials you need to learn. All I ask is that you take a little time out of every week to commit yourself to learning. You will be amazed at how much more you can accomplish in lessons this way! Who wants someone just to teach them notes, anyway? FLEXIBILITY - We will do things in lessons that might seem strange to you at first. We may ask you to make weird mouth shapes while singing or make funny faces/sounds, but there is always a very good teaching reason behind what we do, even if you can’t see it right then (if you’re curious, ask!). Although you will have sometimes have a choice in songs, I will also assign you things to learn, sing, and/or perform that I feel is the best choice for getting you to the next level in singing ability. Try to create for yourself an attitude of open-mindedness in trying new things with singing. Everyone has something to learn! DEPENDABILITY - Let’s be honest: we all get nervous at some point or another. Many people have to overcome fear of singing in front of others. But that is no reason to miss a performance, especially because one of the only paths to conquering that fear is by trying to sing in front of others. If I put you in a recital or program, that means I trust you to be there. If you miss a performance that you are supposed to be at for any reason, Ms. Delaney will schedule a time for you to sing your song in front of your choir class the next day or week you are in school. Please know that this is the only way to ensure that you are getting the experience you need in order to become a more successful singer. STUDENTS By signing this, I (the student) agree to PRACTICE every week, be FLEXIBLE, and be DEPENDABLE for my voice lesson teacher.
