This submittal consists of two individual reports to fulfill the reporting requirements of our NPDES Permit as noted below. The reports are as follows: 1. Inflow Removal Program Report 2. Salem's Management, Operation, and Maintenance (sMOM) Program Report NPDES Permit No. 101145, Schedule B.3, Report Submittals, states, "The permilfee shall implement the Inflow Removal Plan required in ScheduleD, Condition 1, and the }vfanagement, Operation and kfaintenance Program spec[{ted in ScheduleD, Condition 9, to ident[fy and reduce inflow and il?filtration into the sewage collection system. An annual report shall be submitted to the Department by September 1 ofeach year, which demonstrates compliance with the above requirements. The City received notification by letter elated May 5, 2005, that the Department compl eted review of the City's previously submitted proposed updated Inflow Removal Program and found it met the goals and intent of the City's NPDES Permit. The Department letter further indicated that the finalized proposal required submission to the Department by no later than July 5, 2005. On June 29, 2005, the City submitted to the Department for approval the final updated Inflow Removal Program and Time Schedule. Notification was received by letter elated September 5, 2008, of the Department's approval of the final updated Inflow Removal Program and Time Schedule. NPDES Permit No. 101145, ScheduleD- Special Conditions, Condition 9.b.viii, Program Report, states, "As required by Schedule B, Condition 3.a. of the NPDES Permit, the permittee must conduct an evaluation, appropriate to the size of the system and the number of ove1jlows, and submit a report ofsuch evaluation, evaluating sMOM and its compliance with these requirements, including its deficiencies and steps to respond to them." The initial evaluation report of Salem's wastewater collection system under the new NPDES Permit criteria covering the sMOM Program was submitted as part of the FY 2004-05 Annual Report for the City of Salem. The FY 2005-06 Annual Report updated the initial evaluation of the wastewater collection system as warranted and subsequent reports continue the annual process of updates to the initial evaluation. The City of Salem continues its efforts to reduce the amount of wastewater collection system inflow and infiltration, as well as the frequency of wastewater collection system overflows. The City is required to comply with the conditions stated in Condition 1 of Schedule C - Compliance Schedules and Conditions, and Condition 1 of Schedule D - Special Conditions of Salem’s NPDES Permit. Each permit condition related to the Inflow Removal Program is listed separately along with the City’s plan for compliance and the associated time schedule. REGULATORY REQUIREMENTS NATIONAL POLLUTANT DISCHARGE ELIMINATION SYSTEM WASTE DISCHARGE PERMIT Expiration Date: 10-31-2009 (Administratively Extended) Permit Number: 101145 File Number: 78140 SCHEDULE C Compliance Schedules and Conditions 1. By May 5, 2005, the permittee shall submit to the Department for review and approval a proposed updated program and time schedule for identifying and reducing inflow. Within 60 days of receiving written Department comments, the permittee shall submit a final approvable program and time schedule. The program shall consist of the following: a. Identification of all overflow points and verification that sewer system overflows are not occurring up to a 24-hour, 5-year storm event or equivalent; b. Monitoring of all pump station overflow points; c. A program for identifying and removing all inflow sources into the permittee's sewer system over which the permittee has legal control; and d. If the permittee does not have the necessary legal authority for all portions of the sewer system or treatment facility, a program and schedule for gaining legal authority to require inflow reduction and a program and schedule for removing inflow sources. FY 2012-13 INFLOW REMOVAL PROGRAM REPORT 1 SCHEDULE D Special Conditions [Program Goal] 1. Unless otherwise approved in writing by the Department, all inflow sources are to be permanently disconnected from the sanitary sewer system in accordance with the program for identifying and removing all inflow sources required by Schedule C, Condition 1. PROGRAM COMPLIANCE On May 2, 2005, the City submitted to the Department for review and approval the proposed updated Inflow Removal Program and time schedule to comply with the conditions stated in Condition 1 of Schedule C - Compliance Schedules and Conditions, and Condition 1 of Schedule D - Special Conditions of Salem’s NPDES Permit. Notification was received by letter dated May 5, 2005, that the Department had completed review of Salem’s previously submitted proposed Updated Inflow Removal Program and found it met the goals and intent of Salem’s NPDES Permit. The Department letter further indicated the finalized proposal required submission to the Department by no later than July 5, 2005. On June 29, 2005 the City submitted to the Department for approval the Final Updated Inflow Removal Program and time schedule. Notification was received by letter dated September 5, 2008, of the Department’s final approval of the Final Updated Inflow Removal Program and Time Schedule. Permit conditions noted in this section are shown with underlining. The City’s plan and/or response for each specific permit condition is shown italicized following the underlined permit condition. 1.a. Identification of all overflow points and verification that sewer system overflows are not occurring up to a 24-hour, 5-year storm event or equivalent. The City continues to conduct an on-going computer modeling effort to improve and refine its understanding of the wastewater collection system. The modeling helps to determine existing system capacity, planning and development of capacity upgrades for future growth and elimination of SSOs (see Attachment 2: Fiscal Years 2012-13 Through 2016-17 Capital Improvement Plan Wastewater Projects), and provides identification of potential overflow locations due to the 24-hour, 5-year storm event. Much of this refinement continues to come about through updating, correcting, and expanding the XP-SWMM computer hydraulic model of the wastewater collection system. Field verification of system flows is accomplished through the use of permanent and temporary flow monitors/data collectors. The monitors provide both flow and depth data, which verifies system flows for various storm events and validates that sewer system overflows are not occurring for up to a 24-hour, 5-year storm event. The location was: MANHOLE LOCATION SANITARY SEWER BASIN Union Street Overflow Willamette River The SSO during the 2012-13 winter was caused by a rapid and prolonged increase in flow within the wastewater system during a wet weather event, causing an overflow at a designated NPDES site. The City has conducted engineering evaluations of this particular overflow site, as well as in the surrounding collection system, consisting of field inspections, site visits, research of past records, and hydraulic modeling. Alternatives to correct any identified system deficiencies will be developed and included in the City’s CIP process as appropriate. A schedule to eliminate the overflows which appeared during 2005-06, 2006-07, and 2007-08 had been developed after the completion of the engineering evaluation and adoption of the recommendations. Capital improvement projects to correct these overflows have been completed. The City’s 5-year CIP funding schedule beginning FY 2012-13 includes a single project to address a recurring SSO site at Lansing/Chester. That project is scheduled for construction in FY 2013-14. STATUS Monitoring of all pump station overflow points. The City continues to utilize a Supervisory Control and Data Acquisition (SCADA) system for all of the wastewater collection system pump stations. SCADA allows for computerized remote monitoring and controlling of pumping systems. Information from each sewage pump station is transmitted through a radio or telephone communications system to the main SCADA system. The data is processed by the SCADA system and displayed through computer screens to allow operators to view the wet well level, pump status, flows, etc. The level sensing devices located in each sewage pump station wet well are set to activate should the wet well level rise to a point that would result in an overflow condition. Upon activation, a signal is sent to the SCADA system which in turn generates an alarm signal notifying the operator of the abnormal condition. The City continues to satisfy this requirement, and as such, it does not have an associated time schedule for completion. FY 2012-13 INFLOW REMOVAL PROGRAM REPORT 5 1.c. A program for identifying and removing all inflow sources into the permittee's sewer system over which the permittee has legal control. Wastewater collection system maintenance activities are conducted each year to keep the system operating properly, extend its life, and prevent, identify, and remove sources of inflow and infiltration. Maintenance activities performed during FY 2012-13 and planned for FY 2013-14 are found in Attachment 1: Sanitary Sewer Maintenance Activities. The City continues to conduct an on-going program of smoke testing of the wastewater collection system to inspect for and/or confirm the presence of improper connections resulting in inflow. Smoke testing of approximately 10,000 lineal feet of pipe is conducted on an annual basis. Defects found are noted and referred for appropriate corrective action. Corrective actions may include replacement of missing clean-out caps and removal of illicit connections (rain drains, foundation drains, area drains, and basement stormwater sump pump discharges) from the wastewater collection system. The Salem Revised Code includes provisions requiring property owners to make corrections needed to eliminate inflow due to illicit connections. The City inspects all sanitary sewer service lines when installed or repaired. All sanitary sewer hookups are required to pass a 15-minute no-loss air test or hydrostatic test in conformance with the Uniform Plumbing Code (UPC) before they are accepted. No-loss requirements are strictly enforced to prevent infiltration/inflow from entering the wastewater collection system via new sanitary sewer service lines. Storm drainage connections are also inspected to assure that no illicit connections are made to the wastewater collection system and that the stormwater drainage systems are properly installed. Closed circuit television (CCTV) inspections are completed on a routine basis, and defects found are noted and referred for appropriate corrective action. Corrective actions may include pipe replacement, rehabilitation, and repair. The City continues to satisfy this requirement, and as such, it does not have an associated time schedule for completion. FY 2012-13 INFLOW REMOVAL PROGRAM REPORT 6 1.d. If the permittee does not have the necessary legal authority for all portions of the sewer system or treatment facility, a program and schedule for gaining legal authority to require inflow reduction and a program and schedule for removing inflow sources. The City of Salem’s wastewater collection system continues to be operated under the provisions of Chapter 73 of the Salem Revised Code. This chapter provides the legal authority to require inflow reduction within both the public portion of the system and the privately owned service laterals. The City of Salem also operates and maintains the wastewater collection systems for the following cities and districts by intergovernmental agreement: City of Keizer, Oregon City of Turner, Oregon East Salem Service District, Marion County, Oregon Labish Village Sewer and Drainage District, Marion County, Oregon The intergovernmental agreements include provisions requiring each city and district to take all necessary steps to adopt, keep current, and enforce rules and regulations concerning the collection and disposal of wastewater within the city/district. Such rules and regulations shall be in substantial conformity with all applicable ordinances of Salem, including, but not limited to those included in Salem Revised Code Chapter 73 Sewers. Should city/district fail or refuse to adopt rules and regulations as provided for in the intergovernmental agreement, such failure or refusal shall be grounds for termination of the intergovernmental agreement. The City continues to satisfy this requirement, and as such, it does not have an associated time schedule for completion. FY 2012-13 INFLOW REMOVAL PROGRAM REPORT 7 EXHIBIT 2 SALEM'S MANAGEMENT, OPERATION, AND MAINTENANCE (sMOM) PROGRAM REPORT SALEM'S MANAGEMENT, OPERATION, AND MAINTENANCE (sMOM) PROGRAM REPORT INTRODUCTION This report represents the eighth annual program review since the initial FY 2004-05 evaluation of Salem's wastewater collection system under the new NPDES Permit criteria covering Salem's Management, Operation, and Maintenance (sMOM) Program. This report summarizes the status of Salem's Management, Operation, and Maintenance (sMOM) Program compliance. REGULATORY REQUIREMENTS NATIONAL POLLUTANT DISCHARGE ELIMINATION SYSTEM WASTE DISCHARGE PERMIT Expiration Date: 10-31-2009 (Administratively Extended) Permit Number: 101145 File Number: 78140 NPDES Permit No. 101145, Schedule D Special Conditions, Condition 9.b.viii Program Report, states "As required by Schedule B, Condition 3.a. of the NPDES Permit, the permittee must conduct an evaluation, appropriate to the size of the system and the number of overflows, and submit a report of such evaluation, evaluating sMOM and its compliance with these requirements, including its deficiencies and steps to respond to them." SCHEDULE D Special Conditions 9. Salem's Management, Operation, and Maintenance (sMOM) Program (a) General Standards. The permittee shall implement this program to reduce raw sewage overflows from November 1 through May 21 to those due to storms greater than the one-in-five year, 24-hour duration storm. The Permittee must comply as soon as practicable and not later than December 31, 2009, or completion of all upgraded and new facilities required in paragraph 12.C.4 of the Mutual Agreement and Order #WQ/M-WR-97-147 whichever is earlier. The Permittee must: (i) Properly manage, operate, and maintain, at all times, all parts of the collection system that the Permittee owns or over which they have operational control; (ii) Provide adequate capacity to convey base flows and peak flows resulting from up to a 24-hour 5-year storm event (or equivalent) for all parts of the collection system they own or over which they have operational control; (iii) Take all economically feasible steps to stop, and mitigate the impact of, sanitary sewer overflows resulting from up to a 24-hour 5-year storm event (or FY 2012-13 SALEM’S MANAGEMENT, OPERATION AND MAINTENANCE (sMOM) PROGAM REPORT 1 (b) equivalent) in portions of the collection system they own or over which they have operational control; (iv) Provide notification to parties with a reasonable potential for exposure to pollutants associated with the overflow event; and (v) Develop a written summary of the sMOM program and make it and the annual collection system report available to any member of the public upon request. Components of sMOM Program. The permittee must develop and implement a management, operation, and maintenance program to comply with the requirements of this section. The Director will consider the quality of the sMOM program, its implementation and effectiveness in any relevant enforcement action, including but not limited to any enforcement action for violation of the prohibition of any municipal sanitary sewer system discharges resulting from up to a 24-hour 5-year storm event (or equivalent) described at paragraph (1) (ii) of this section. The program must include the following components, with the exception of non-applicable components as discussed above: (i) Goals. The permittee must specifically identify the major goals of the sMOM program, consistent with the general standards identified above. (ii) Organization. The permittee must identify: (A) Administrative and maintenance positions responsible for implementing measures in the sMOM program, including lines of authority by organization chart or similar document; and (B) The chain of communication for reporting SSOs from receipt of a complaint or other information to the person responsible for reporting to the NPDES authority, or where necessary, the public. (iii) Legal Authority. The permittee must include legal authority, through sewer use ordinances, service agreements or other legally binding documents, to: (A) Control infiltration and connections from inflow sources; (B) Require that sewers and connections be properly designed and constructed; (C) Ensure proper installation, testing, and inspection of new and rehabilitated sewers (such as new or rehabilitated collector sewers and new or rehabilitated service laterals); and (D) Implement the general and specific prohibitions of the national pretreatment program that the Permittee is subject to under 40 CFR 403.5. (iv) Measures and Activities. The sMOM program must address the following elements that are appropriate and applicable to the system and identify the person or position in the organization responsible for each element: (A) Provide adequate maintenance facilities and equipment; (B) Maintenance of a map of the collection system; (C) Management of information and use of timely, relevant information to establish and prioritize appropriate sMOM activities (such as the immediate elimination of dry weather overflows or overflows into sensitive waters such as public drinking water supplies and their source waters, swimming beaches and waters where swimming occurs, shellfish beds, designated Outstanding National Resource Waters, National Marine Sanctuaries, waters within Federal, State, or local parks, and water containing threatened or endangered species or their habitat), and identify and illustrate trends in overflows, such as frequency and volume; (D) Routine preventive operation and maintenance activities; FY 2012-13 SALEM’S MANAGEMENT, OPERATION AND MAINTENANCE (sMOM) PROGAM REPORT 2 (E) A program to assess the current capacity of the collection system and treatment facilities, which the Permittee owns or over which they have operational control; (F) Identification and prioritization of structural deficiencies and identification and implementation of short-term and long-term rehabilitation actions to address each deficiency; (G) Appropriate training on a regular basis; and (H) Equipment and replacement parts inventories including identification of critical replacement parts. (v) Design and Performance Provisions. The permittee must establish: (A) Requirements and standards for the installation of new sewers, pumps and other appurtenances; and rehabilitation and repair projects; and (B) Procedures and specifications for inspecting and testing the installation of new sewers, pumps, and other appurtenances and for rehabilitation and repair projects. (vi) Monitoring, Measurement, and Program Modifications. The permittee must: (A) Monitor the implementation and, where appropriate, measure the effectiveness of each element of the sMOM program; (B) Update program elements as appropriate based on monitoring or performance evaluations; and (C) Modify the summary of the sMOM program as appropriate to keep it updated and accurate. (vii) Overflow Emergency Response Plan. The permittee must develop and implement an overflow emergency response plan that identifies measures to protect public health and the environment. The plan must include mechanisms to: (A) Ensure that appropriate staff is made aware of all overflows (to the greatest extent possible); (B) Ensure that overflows (including those that do not discharge to waters of the U.S.) are appropriately responded to, including ensuring that reports of overflows are immediately dispatched to appropriate personnel for investigation and appropriate response; (C) Ensure appropriate immediate notification to the public, health agencies, other impacted entities (e.g., water suppliers) and the NPDES authority. The sMOM program should identify the public health and other officials who will receive immediate notification; (D) Ensure that appropriate personnel are aware of and follow the plan and are appropriately trained; and (E) Provide emergency operations. (viii) sMOM Program Report. As required by Schedule B, Condition 3.a. of the NPDES Permit, the permittee must conduct an evaluation, appropriate to the size of the system and the number of overflows, and submit a report of such evaluation, evaluating sMOM and its compliance with these requirements, including its deficiencies and steps to respond to them. FY 2012-13 SALEM’S MANAGEMENT, OPERATION AND MAINTENANCE (sMOM) PROGAM REPORT 3 PROGRAM COMPLIANCE Permit conditions noted in this section are shown with underlining. The City plan and/or response for each specific permit condition and the level of achievement is noted in bold italics following each criterion. (vi) Monitoring, Measurement, and Program Modifications. The permittee must: (A) Monitor the implementation and, where appropriate, measure the effectiveness of each element of the sMOM program; Wastewater Collection Services conducts on-going monitoring of the collection system performance to determine compliance with the requirements of sMOM. During FY 2009-10 Wastewater Collection Services completed a study of the various maintenance programs to determine their effectiveness in preventing SSOs from occurring. At the conclusion of the study, recommendations were made addressing any identified changes regarding maintenance practices, use of technology, staffing levels, equipment needs, outsourcing needs, etc. Several of these recommendations were implemented in FY2010-11 and evaluation of its effectiveness continued through FY2012-13. Determination of the effectiveness of the sMOM program is based upon how well the City is meeting the following sMOM program goals. 9.(a)(i). Properly manage, operate, and maintain, at all times, all parts of the wastewater collection system that the City of Salem owns or over which Salem has operational control; Criteria: (a) Are there any pipeline failures (collapses) that result in an SSO? None identified. (b) Are there any pump station failures that result in an SSO? None identified. (c) Are there any non-storm related SSOs resulting from causes other than vandalism? Yes; the cause of the overflow event and number of similar occurrences are as follows. Details of these events can be found in Attachment 3, Sewer Bypass and Overflow Report. Take all economically feasible steps to stop, and mitigate the impact of, sanitary sewer overflows resulting from up to a 24-hour 5-year storm event (or equivalent) in portions of the wastewater collection system that the City of Salem owns or over which Salem has operational control; FY 2012-13 SALEM’S MANAGEMENT, OPERATION AND MAINTENANCE (sMOM) PROGAM REPORT 4 Criteria: (a) Are the facilities identified in the NPDES Permit and the MAO needed to convey and treat the system flows resulting from up to a 24-hour 5-year storm event being implemented as agreed to with the Department? Yes; The requirements of Mutual Agreement and Order No. WQ/MWR-97-147 have been fulfilled, and the MAO was terminated on March 1, 2011. 9.(a)(iv). Provide notification to parties with a reasonable potential for exposure to pollutants associated with the overflow event; and Criteria: (a) Are all parties identified in the Overflow Emergency Response Plan being notified as required? Yes. 9.(a)(v). Develop a written summary of the sMOM program and make it and the annual collection system report available to any member of the public upon request. (B) Update program elements as appropriate based on monitoring or performance evaluations; Criteria: (a) Wastewater Collection Services will conduct an annual update to the sMOM program elements as appropriate based on on-going monitoring and evaluation of collection system performance. This is completed on an annual basis. (C) Modify the summary of the sMOM program as appropriate to keep it updated and accurate. Criteria: (a) Wastewater Collection Services will modify and/or update the sMOM summary program report as appropriate based on on-going monitoring and evaluation of collection system performance. This is completed on an annual basis. FY 2012-13 SALEM’S MANAGEMENT, OPERATION AND MAINTENANCE (sMOM) PROGAM REPORT 5 CONCLUSIONS The City has successfully fulfilled the requirements specified in the MAO, and continues to meet the majority of the sMOM program objectives. As such, it was determined that the cause was due to recent rainfall and saturated soils. The City is conducting further inspections to determine to what extent this overflow was caused by a capacity issue. Nevertheless, a more frequent cleaning and inspection frequency of the system’s large diameter pipes are being scheduled. Non-storm related sanitary sewer overflows (SSOs) still periodically occur within the wastewater collection system. These have typically been associated with grease buildup and/or root intrusion problems. The City continues its comprehensive fats, oils, and grease (FOG) program, which includes a Web page to educate the public of the potential consequences household grease has on the sewer system, public health, and the environment. The Web page is linked to the City’s main Web site and provides the public resources for reducing fats, oils, and grease from entering the sewer system and creating backups. A pro-active notification and verification process is progressing to notify property owners of potential grease buildup in their service lateral when observed during routine TV inspection of the City’s mainline assets. Ongoing evaluation of grease maintenance activities have provided strategies to effectively reduce the potential of grease backups – instead of hydrocleaning known grease lines, crews automatically brush and cut mainlines known to have grease buildup. Cleaning frequencies of these lines are regularly evaluated for appropriate maintenance schedules. With further development, implementation, and FY 2012-13 SALEM’S MANAGEMENT, OPERATION AND MAINTENANCE (sMOM) PROGAM REPORT 6 evaluation of these newly implemented programs, the City expects to eliminate most of the SSOs caused by grease buildup. (2) During the 2005-06 winter the wastewater collection system experienced overflows at several locations due to heavy rainfall. Both lines were in the street out front of these addresses and the licensed plumber and onsite inspector failed to insure there services were connected to the proper line. City crews TV'ed and dye tested the services to verify there was a sanitary sewer cross connection to the stom sewer. Flow from the affected storm line discharges into a downstream stormwater detention basin. The contractor was contacted and told they need to repair the cross connection, this will take time due to permit processing. The storm line was flushed out to the detention basin and the line will be thouroughly cleaned after the repairs have been made. No signs or samples were taken since the flow was contained in the detention basin and did not impact Waln Creek. OERS was notified(OERS #2012-2944) as soon as the problem was identified. While onsite, John Duran, local water purveyor for the Labish Village Water Commission(503-586-4956) expressed concerns about their 4 private wells that provide drinking water to the home in Labish Village. He was told there should not be a concern regarding his wells but he said thay are fairly shallow, about 80 feet deep. He was told to contact Risk Management. He called again after midnight and wanted Environmental Services to stop the flow syphoning back from the mile long force main, I told him there was nothing I could do and the crews onsite would do what they could, he wasn't real happy about the situation and he was told to contact Risjk Management and that the contractor was taking full responsibility for the incident and if Labish Village had any bad water tests due to the sewage spill he could pursue that with them. He was going to have the one well nearest the break tested at Waterlab and if he needed to test the others he was going to pass this cost onto Mountain West. First thing on Friday morning I was able to meet with MC PW and they showed me a map indicating the storm drain drained east to the farm field and than south to Labish Ditch near the address of 6345 Lakeside Dr NE.
