BACKGROUND
One of the main challenges of Tampere health region is the migration of different medical
data repositories towards standard, shareable formats. Currently information like medical
images is stored into data repositories that are vendor and/or system specific and make the
sharing of the medical information between organisations and systems difficult or
impossible within the region. This creates significant expense when integrating systems
within and between organisations. It also creates significant expenses when migrating
medical applications as medical data is stored in a vendor specific way and difficult to
migrate to new medical applications.
Open standards are key to avoid proprietary information system implementations. When
used to develop information management systems, the standards enable information
sharing between systems and organisations. Especially IHE’s (Integrating the Healthcare
Enterprise initiative) XDS (Cross Enterprise Document Sharing) standard and its’ imaging
focused specification (XDS-I) make it possible to store and share imaging information based
on a generally accepted standard.
Fujitsu Services, in close collaboration with EMC, has developed a solution based on the IHE
XDS standard, where various imaging related systems (such as PACS, Picture Archive
Communication System) can be integrated in controlled phases to utilize a standard, vendor
independent information management system called Archive Service for Health Care
Imaging.
SOLUTION
Tampere Region in Finland selected the EMC Open Systems Architecture (OSA) and Fujitsu to
build a centralized regional medical archiving and data-sharing infrastructure for medical
images based on the XDS (Cross-Enterprise Document Sharing) technology. In a first phase
the 3 main hospital PACS (Picture Archiving and Communication Systems) applications
producing over 260,000 medical imaging exams will be connected to the Tampere XDS
archive. In the second phase all medical imaging producing systems in the Tampere will be
connected to the EMC Open Systems Architecture sending yearly up to 1 million medical
imaging exams in the XDS archive. This represents an annual archiving requirement of
100TB net capacity to be hosted in 3 data centres. The whole solution will be centrally
hosted in the data-centres of Fujitsu Services in Tampere and Helsinki and invoiced by
Fujitsu Services as a service (annual fee per exam) to the region. The EMC Open Systems
Architecture (OSA) solution is based on VMware, the EMC Documentum platform, EMC
Documentum xDB, EMC Centera and EMC Professional Services. It is also the first win
globally for EMC that includes the XDS technology components (EMC Documentum as XDS
repository and EMC xDB as XDS registry) built by the EMC IIG product division.

The Archive Service for Health Care Imaging is offered by default as a SaaS service. Different
imaging related systems (such as PACS, Picture Archive Communication Systems) utilize the
solution, as a Long Term Archive to archive health care imaging related information. It
provides services to store, publish, search and retrieve imaging related documents, such as
imaging studies (images, measurements and presentation states), diagnostic reports and
key object selections. Imaging documents are stored following XDS-I standards into the
archive and retrieved according to operations performed in imaging systems. The FujitsuEMC solution provides a scalable, efficient environment to archive imaging information
based on vendor independent XDS-I standard (including DICOM and HL7).
In addition to the image archive services, the solution can be extended to various other
archiving needs. It offers interfaces to utilize for example Web Services and Java
technologies. Also, the solution can be extended to a full scale XDS implementation and be
used for archiving non-XDS information (such as word processing documents and
spreadsheets).
The commercial model of the service is based on volumes. The model is simple and enables
customer to have reliable calculations of total cost of ownership. Fujitsu Services takes
responsibility for all related services according to agreed service level.

The combination of EMC’s healthcare technology portfolio and Fujitsu’s systems integration
and outsourcing services means that Tampere region benefits from a medical imaging
archive that provides access to patient information across modalities, hospitals and clinical
disciplines. Not only does the Open Systems Archive provide a better service but it will offer
considerable cost benefits over time, which aligns the use of the service with the cost to
Tampere region.
