DEPARTMENT OF TECHNOLOGY, MANAGEMENT AND BUDGET, DIMONDALE - Secondary Governmental Complex, Energy Center - Cooling Tower Replacement, Phase 3 File No. 071/13033.DCS -Index No. 43204 - Contract No. Y 13220 Recommended Contract Award: EVR Incorporated, Okemos; $3,085,000.00 Description and Justification The purpose of this contract is to provide for the complete demolition, removal, and replacement of the two (2) original cooling towers and associated structural, piping, and electrical scope at the State Secondary Governmental Complex, Energy Center. These two (2) existing cooling towers are an integral part of the overall chilled water system serving the entire State Secondary Governmental Complex. DEPARTMENT OF MILITARY AND VETERANS AFFAIRS, GRAND RAPIDSGrand Rapids Home for Veterans - Exterior Window Replacement File No. 511/12303.GAG - Index No. 65000 - Contract No. Y 13221 Recommended Contract Award: DAV Carrier Group, LLC, Hickory Corners; $967,000.00 Description and Justification The purpose of this contract is to remove and replace specified windows and doors in the four floors of the McLeish, Main-1/Courtyard, Chapel, and Connector Way buildings at the Grand Rapids Home for Veterans. This includes new window openings with new steel lintels and steel columns in the dining/day rooms on the second, third, and fourth, floors of the McLeish BUilding necessary to meet window square footage requirements 1 05/23/2013 - 3:25 pm - FINAL June 4, 2013 No.5 in members' areas. This project will involve replacing approximately 450 windows of varying sizes and configurations throughout the included facilities. All windows units are to be sealed and insulated, with thermally broken and internally reinforced extruded aluminum framing, and low "E" tinted or reflective glazing. DEPARTMENT OF TRANSPORTATION, DETROIT - Detroit (Greyhound) Bus Terminal - Renovation Project File No. 591/13105.AGY -Index No. 75600· Contract No. Y 13345 Low Responsive Bidder: Freedom Contracting Group, LLC, New Hudson; $320,350.00 Description and Justification The purpose of this contract is to renovate the existing restrooms, lobby seating and baggage storage areas, retro-fit the existing light fixtures to accept LED bulbs, provide landscape stone, and bituminous parking lot resurfacing. This work will provide ADA code compliance, reduced maintenance and operational costs, and for preserve the interigty of the facility. Funding Source 100% Trunkline Funds 4. DEPARTMENT OF NATURAL RESOURCES, HOllY - Holly Recreation AreaCampground and Day Use Road Restoration - Phase I File No. 751/12181.BDW -Index No. 59700 - Contract No. Y 13222 Recommended Contract Award: Service Disabled Qualified Veteran General Contracting, Inc., Eaton Rapids; $830,440.00 Description and Justification The purpose of this contract is to repave sections of the campground loops, demolish and reconstruct the sanitation station, repave the sections of the day use area roadways/parking and construct a second (back) entrance drive. The existing pavement is in poor to very poor condition, in some locations, it was milled back to gravel. The poor roadbed requires significant maintenance and causes drivers to leave their lane to avoid damaged road edges. The existing sanitation station does not provide for efficient staging of vehicles and causes significant delays and traffic backups. The proposed second entrance at the back of the campground will provide for the elimination of three quarters of a mile of campground road and better control of the traffic to the Wildwood Lake Camper Cabins. DEPARTMENT OF ENVIRONMENTAL QUALITY, SPRING ARBOR - Spring Arbor Wash N' Dry Site - Source Area Removal File No. 761/13037.BDH -Index No. 44401 - Contract No. Y 13223 2 05/23/2013 - 3:25 pm· FINAL June 4, 2013 No.6 Recommended Contract Award: Ripman Construction Company, Inc., St. Helen; $899,257.50 Purpose/Business Case The purpose of this contract is to have the source of contamination removed from this leaking underground storage tank site. The selected contractor wil/excavate and dispose of approximately 6,000 tons of soil contaminated with dry cleaning solvents. An estimated 250,000 gal/ons of groundwater will have to be collected and removed from the site by dewatering. The contractor will also furnish and place backfill in the excavation and otherwise restore the site. The work is needed to comply with the environmental regulations and protect public health, safety and welfare of the environment. DEPARTMENT OF NATURAL RESOURCES, WATERFORD - Dodge No.4 State Park - Boating Access Site Circulation, Parking and Accessibility File No. 751/09294.JAN - Index No. 59090 - Contract No. Y 09387 Beckett and Raeder, Inc., Ann Arbor; Modification No.3, Increase $34,901.61 Description and Justification The purpose of this modification is to include permitting, prepare construction documents for the preferred beach erosion solution as a single phase and reallocate funds. The beach has eroded and based on the previous study a long term stabilization solution has been selected for implementation. DEPARTMENT OF ENVIRONMENTAL QUALITY, REED CITY - Andy's Standard Site - Groundwater Monitoring File No. 761/10307.SAR -Index No. 44301 - Contract No. Y10246 The Mannik & Smith Group, Inc., Canton; Modification No.3, Increase $102,158.55 ISIO No. 00285 - Assignment No. 11 Description and Justification The purpose of this modification is to install additional monitoring wells, conduct in-situ chemical oxidation (ISCO) pilot test, and provide quarterly groundwater sampling and reporting. Previous investigations have confirmed the presence of a dissolved phase petroleum groundwater plume at the site. The original assignment and SUbsequent modifications were to delineate the nature and extent of contamination and identify the most effective remediation technology. It was concluded that additional investigation/monitoring and a pilot study are needed for the site. Under this modification, the professional will redevelop the purge well, replace the pump well, install three new monitoring wells, conduct three injection events of an ISCO reagent and perform eight quarterly groundwater monitoring events. The work is needed to comply with the environmental regulations and protect pUblic health, safety and welfare of the environment. The professional will conduct groundwater sampling, monitoring and analysis and will evaluate the results to determine if additional active remediation is necessary. A groundwater treatment system was installed to clean up the contamination. The professional will perform soil and groundwater investigation, conduct monthly inspections of the treatment system and prepare reports to comply with the environmental regUlations. Previous investigations have confirmed the presence of a dissolved phase petroleum groundwater plume at the site. A groundwater treatment system was installed at the site to control downgradient plume and clean up the contamination. The professional will evaluate in situ chemical oxidation and air sparging, as remedial options to address remaining contamination. DEPARTMENT OF ENVIRONMENTAL QUALITY, TRAVERSE CITY - Stop N Shop Site - Remedial Investigation File No. 761/12059.SAR - Index No. 44301 - Contract No. Y12040 Global Remediation Technologies, Inc., Traverse City; Modification No.2, Increase $61,535.02 ISID No. 320 - Assignment No.1 5 05/23/2013 - 3:25 pm - FINAL June 4, 2013 No.9 Description and Justification The purpose of this modification is to extend the operation and maintenance of the remediation contract twelve months to remove contamination that has proved to be greater in amount than originally anticipated. The professional will provide operation and maintenance of the remediation system, prepare monthly progress reports to be delivered to the Department of Environmental Quality, and furnish eight more activated carbon replacements during the term. The professional will move the remediation system out of the way of a demolition event planned for June of 2013 and set it back up after the demolition. Finally, the professional will perform a soil sampling event to provide data for closure, produce a closure report, permanently close the twelve soil vapor extraction wells and three monitor wells, and demobilize the remediation system. The work is needed to comply with the environmental regulations and protect public health, safety and welfare ofthe environment. DEPARTMENT OF TRANSPORTATION (MOOT), BRIGHTON - Addendum No. 1 to Lease No.1 0483 approved by the State Administrative Board on March 4, 2008, between Fonda Place Partners, LTD, 4132 Marsh Trail, Milford, Michigan 48380, as Lessor, and the State of Michigan by the Department of Technology, Management and Budget for the Department of Transportation, as Lessee, for 10,867 square feet of space located at 10321 East Grand River, Suite 500 Brighton, Michigan, 48116. This Addendum provides for adding one 5-year renewal option. This Addendum is effective upon obtaining State Administrative Board approval and required signatures and continues to the termination date of March 31, 2018, or any extension. The annual per square foot rental rate for this space beginning Apri/1, 2013, is $17.00 ($15,394.92 per month). There are midterm rent increases of approximately 3% each year for four years. This rate does not include electricity, janitorial or replacement of florescent lighting. This Lease contains a Standard cancellation clause with 90 days notice. The operation of this site is a Transportation Service Center, which requires easy access and parking suitable for tractor-trailers for licensing and permit activities. Benefit: This space includes the Brighton Transportation Service Center as well as conference room areas which MDOT uses as a mid-point meeting place for the Southeast and MidMichigan region.
