Enclosed is a listing of property tax forms produced by the Board and available for use by county
assessors' offices and local appeals boards. This listing also contains forms used by Board staff
in the implementation of various property tax programs co-administered by the Board and county
assessors.
Forms included on the list are intended for use throughout the state. Most of the forms are
designed to implement statutory and regulatory requirements. Many of the forms are Boardprescribed. Counties may not use their own forms if there is a Board-prescribed form available.
