10% Site Audit of Asbestos Removal Works ASBESTOS WORK SITE AUDIT FORM V1.0 Overview Project managers should complete a site audit for 10% of their asbestos works. This audit has been designed for use by building professionals without the requirement for specific training. Safety: University staff MUST NOT enter designated respirator zones e.g. contaminated areas, working enclosures, air-lock, bag-lock, waste skips and decontamination units. The audit does not necessitate entry to these areas. The form is appropriate for works that are either licensed or non-licensed (albeit in a reduced capacity). It follows criteria detailed in the ARCA (Asbestos Removal Contractors Association) Site Audit Accreditation Scheme and verifies site works are being undertaken in line with the pre planning stage and on-site method statement/plan of work (PoW). Instruction PMs complete the form for 10% of instructed asbestos works. Comments already provided give further guidance on or what other considerations may be applicable. There should be a named individual in the PoW responsible for the conduct of the site – this supervisor should be able to respond to any questions – if they do not know the answers then assume it is not present/has not been done. Site paperwork can generally be amended on site, as required. It should be accurate at the time of audit – not after it has been requested Completed forms should be held on project file and a copy forwarded to EM Asbestos Team If the site set up appears unsatisfactory or assistance is required, please contact the EM Asbestos Team. The DCU must be fully operational prior to asbestos works starting – including set up. (Section 4 of ASB5) What is the stated maximum number of employees per shift? (Section 5 of ASB5) 2.3 Please copy them in here, or write NO. Look for licensed scaffolder, supervisory licensed holder, labour-only sub-contractors. How many operatives (including the supervisor) are actually on the site? 3.0 Site documentation 3.1 Is the contract supervisor the same person as specified in the plan of work? Must NOT be a temporary employee (NB if the named individual is not there then this is inadequate supervision unless the name is correctly amended in the PoW. Changing PoW whilst you are there is not good enough) 3.2 Does the plan of work detail when the work is to be carried out? (including start / finish times and weekend variations) Does the plan of work detail the type of asbestos involved? (e.g. amosite, chrysotile, crocidolite) Note times 3.3 3.4 Does the plan of work detail the asbestos removal technique to be used? 3.5 Is there a site specific assessment available for inspection, for hazards other than asbestos? Risk assessment is a legal requirement. Note – hot works should not be undertaken without EM Asbestos Team authorisation. COSHH required for spraytack, PU foam, surfactant. (NB Do not be put off by ‘trainee’ operatives there to help build enclosure & without medical, training etc. Reasonable to believe will continue into enclosure/ strip it – liable for HSE prohibition Notice) Current training certificate? Request to see unopened pack of overalls to check e.g. ‘type 5/6’? 8.5 Has the water been appropriately bagged? e.g. immediately bagged, and single bagged if still in enclosure. If not, is it possible? If really not possible without extreme measures, is air monitoring contingency in place? All asbestos removal contractors should provide evidence of a “Removal” licence that is similar in appearance.
