A roll call vote followed: Yeas: Harney, Moy-Sandusky, Penny, Smeelink, Wyett MOTION PREVAILED MOTION by Smeelink, seconded by Harney, to dissolve the Order of Summary Suspension dated September 18, 2014 and suspend Respondent's license for a minimum period of one (1) day. Respondent's license shall be automatically reinstated if, within six (6) months, the Department receives satisfactory written evidence from the Health Professional Recovery Program (HPRP) verifying that the Respondent has undergone a substance abuse/chemical dependency evaluation, has entered into a disciplinary, non-confidential monitoring agreement with HPRP, and that HPRP has endorsed Respondent as safe to practice, OR that HPRP has determined that Respondent does not require treatment monitoring. If Respondent's license remains suspended for more than six (6) months, Respondent shall petition for reinstatement. Additionally, a fine shall be assessed in the amount of $2,500.00, payable prior to reinstatement. Upon automatic reinstatement, Respondent shall be placed on probation to run concurrent with the period of Respondent's HPRP monitoring agreement, if treatment monitoring is required, with the following terms and conditions: (1) Respondent shall comply fully with her HPRP monitoring agreement. If, within six (6) months, Respondent provides evidence that Respondent has updated his address of record with the Department, Respondent's license shall be automatically reinstated. If license remains suspended for more than six (6) months, Respondent must petition for reinstatement, in the matter of Timothy Bos, R.Ph. Michigan Board of Pharmacy Disciplinary Subcommittee Meeting Minutes April 8, 2015 Page 10of10 Discussion was held. A roll call vote followed: Yeas: Harney, Moy-Sandusky, Penny, Smeelink, Wyett MOTION PREVAILED Karlton Mac Wetzsteon, R.Ph - Administrative Complaint MOTION by Penny, seconded by Moy-Sandusky, to suspend Respondent's license for a minimum of one (1) day. If, within six (6) months, Respondent provides evidence of having completed 11.5 hours of Board-accepted continuing education credit, with one (1) hour in the area of pain management, Respondent's license shall be automatically reinstated. If license remains suspended for more than six (6) months, Respondent must petition for reinstatement, in the matter of Karlton Mac Wetzsteon, R.Ph. In addition, Respondent is fined $500.00, to be paid prior to reinstatement. Discussion was held.
