CALL TO ORDER Linda Minter, Chairperson, called the meeting to order at 2:44 p.m. ROLL CALL Members Present: Linda Minter, Public Member, Chairperson Mark Epolito, Public Member Brian Gilbert, PT Ajay Middha, P.T., D.P.T. Barbara Simmons, PTA Members Absent: None Staff Present: Shellayne Grimes, Board Secretary, Policy, Rules & Board Support Pamela Millben, Analyst, Compliance Section Others Present: Vijay Kumar, DPT, Ph.D, MD APPROVAL OF MINUTES MOTION by Simmons, seconded by Middha, to approve the minutes of the January 13, 2015 meeting as presented. MOTION PREVAILED APPROVAL OF AGENDA MOTION by Gilbert, seconded by Epolito, to approve the agenda with the following correction: Grimes noted the removal of item 5. "Master Resolution." A voice vote followed. MOTION PREVAILED REGULATORY CONSIDERATIONS Jeff Philip Burke, P.T. - Consent Order and Stipulation MOTION by Simmons, seconded by Middha, to accept the Consent Order and Stipulation, in the matter of Jeff Philip Burke, P.T. Discussion was held. A roll call vote followed: Yeas - Epolito, Gilbert, Middha, Simmons, Minter MOTION PREVAILED Robertus Hendrikus Sier, P.T. - Consent Order and Stipulation MOTION by Simmons, seconded by Gilbert, to reject the Consent Order and Stipulation and offer a counter-offer to add a course in Ethics to the terms of probation, in the matter of Robertus Hendrikus Sier, P.T. All other terms of the Consent Order and Stipulation to remain as written. Discussion was held. A roll call vote followed: Yeas - Epolito, Gilbert, Middha, Simmons, Minter MOTION PREVAILED Jordan David Stockman, P.T.A. - Consent Order and Stipulation MOTION by Middha, seconded by Gilbert, to accept the Consent Order and Stipulation, in the matter of Jordan David Stockman, P.T.A. Michigan Board of Physical Therapy Disciplinary Subcommittee Meeting Minutes April 14, 2015 Page 3 of 4 Discussion was held. A roll call vote followed: Yeas - Epolito, Gilbert, Middha, Minter Simmons Recuse~ MOTION PREVAILED Shahzad Iqbal Mirza, P.T. - Administrative Complaint MOTION by Simmons, seconded by Middha, to dissolve the Order of Summary · Suspension and suspend Respondent's license for a minimum of one (1) day until Respondent undergoes an HPRP evaluation and enters into a monitoring agreement, if needed, in the matter of Shahzad Iqbal Mirza, P.T. Respondent's license shall be automatically reinstated when the Department receives satisfactory evidence from the HPRP verifying either that (1) Respondent has entered into a disciplinary confidential monitoring agreement with the HPRP and that the HPRP has endorsed Respondent as safe to practice, or that (2) the HPRP has determined that Respondent does not require treatment monitoring. Upon automatic reinstatement, Respondent shall be placed on probation for one (1) year with the following terms and conditions: (1) Respondent shall be required to complete a minimum of three (3) credits of continuing education in the following area: a. Ethics Respondent shall comply with the HPRP, as necessary. Additionally, a fine shall be assessed in the amount of $250.00, payable prior to application for reinstatement or relicensure. · Discussion was held.
