immunizations. The
process reviews not only
vaccine coverage levels,
but also the challenges
and barriers occurring at
practices. A quality
improvement plan is
There are four categories (2 pediatric and 2 adolescent) for the awards. The providers listed have met or exceeded the category criteria that are listed below.
The data was retrieved from the Michigan Care Improvement Registry (MCIR) as
of June 2016. Staff from these offices, attending the 2016 conferences, will be
given green AFIX Award ribbons to wear on their name tags and the practice/
clinic will receive a framed certificate to display in their waiting area. If you are
attending one of the conferences and you see someone wearing the green ribbon, please congratulate them and ask them a question about their immunization activities and strategies in their practice that have resulted in high coverage
levels.
