We are excited at the prospects of getting to know you more on a personal basis. Residency will provide you with the opportunity to integrate knowledge, skills and interpersonal traits towards your ultimate goal in your chosen area of specialization. Our PGME team will work closely with your Residency Program Director/team in making certain that your educational concerns will be met in a timely manner. Do not hesitate to utilize our services or to contact us if you have any concerns. Please find in this Registration Manual for New Postgraduate Trainees information that will help you during your first few weeks at the University of Manitoba. Once again, welcome to the Postgraduate Medical Education at the University of Manitoba. The PGME Team Back to Table of Contents 3 Postgraduate Medical Education Introduction As you may already know, Postgraduate Medical Education (PGME) is unique in that while all the programs are supported and monitored by the University of Manitoba and the Faculty of Medicine, the regulations governing training, evaluation and certification are set by the two national institutions- The Royal College of Physicians and Surgeons of Canada and The College of Family Physicians of Canada. Each program has a Program Director and Program Assistant. These individuals will become known to you during your orientation sessions within your own program. The Program Director is responsible for ensuring that the training guidelines from the two colleges are followed. Each program, regardless of size or function, has a Residency Program Committee (RPC). These committees have resident members and you should find out who your representative(s) are on these committees. The RPCs are responsible for dealing with educational issues in each program. Most programs also have a chief resident who is in his/her final years of training. The chief resident has a variety of duties, some of which may include setting up call schedules and arranging educational sessions. For WRHA funded residents, the Professional Association of Resident and Intern of Manitoba (PARIM) is your association representing both educational and service issues within and around the boundaries of the Faculty of Medicine. The PARIM office is located at the HSC campus. The organization has an Executive and Board of Directors. In addition to the Resident Program Committees in each program, there is a faculty organization. The Faculty PGME executive Committee, chaired by the Associate Dean for Postgraduate Medical Education, is a working committee which meets monthly to discuss educational issues, assign residency positions, formulate policies etc. There are a number of subcommittees that deal with education development, core curriculum, basic clinical year and resident transfers that report to the Faculty PGME Committee. The resident representatives to all these committees are appointed by the PARIM Executive. This email will include the requirements and information you are required to complete which will facilitate your registration in Postgraduate Medical Education at the University of Manitoba. Your primary contacts within the Faculty of Medicine Postgraduate Medical Education (FPGME) office will be the Resident Administrators who will endeavor to address your needs effectively and professionally. Registration Process April 2014 FPGME Resident Administrator Send Letter of Offer to New Trainees Trainee Trainee will be required to: 1. Review, Sign, Scan & Email the Letter of Offer back to the FPGME office at [email protected] 2. Submit the ORIGINAL Signed Letter of Offer to the FPGME Office prior to start of training. Back to Table of Contents 8 A. Banking Information: o All postgraduate medical trainees are required to submit their banking information so that they can be set up with payroll. Voided Cheque or Account Verification from the bank must be submitted to the PGME Office One Month prior to the training start date. o The Voided Cheque must include bank name, address, account number, and transit number. o Sponsored (apply to Saudi Arabian only) - are required to submit their banking information to the PGME Office. B. Canadian Post-M.D Education Registry (CAPER): o All postgraduate medical trainees are required to submit the CAPER form to the PGME Office. o The mandate of the CAPER is: to provide accurate information regarding the size, nature and distribution of post-M.D. programs in Canadian medical schools. to develop a common classification system of post-M.D. trainee categories and to identify such trainees in the Canadian medical schools. to provide information as specifically requested in keeping with CAPER policies. o CAPER form. (on the PGME Resources website – under letter “C” on the right hand side column) C. Canadian Medical Protective Association (CMPA): o All postgraduate medical trainees whose work involves patient contact must obtain CMPA membership/its equivalent for the duration of their registration with PGME at the University of Manitoba. The PGME Office has provided the CMPA with a listing of all trainees accepted to the University of Manitoba for postgraduate medical education training. o It is the trainee's responsibility to complete the on-line membership application form and PGY questionnaire. The CMPA application and questionnaire must be completed and returned to the CMPA by May 16th, 2014. o The CMPA application and questionnaire are available on the CMPA website @ www.cmpa-acpm.ca/en/member-self-service. Back to Table of Contents 9 o CMPA is required prior to the trainee's training start date. Failure to complete/submit the CMPA membership application by the posted deadline may result in a delayed start to the training. o WRHA Residents - the membership fees will be paid in full directly to the CMPA by the Winnipeg Regional Health Authority (WRHA). DO NOT provide any payment when submitting the CMPA application, and be sure to indicate that the WRHA is the third party payer. The portion of membership fees will be recovered by way of payoll deduction, and this will be coordinated by the WRHA or Health Sciences Centre (HSC) Payroll Office in August/September. o Professional Association Residents & Interns of Manitoba (PARIM) - CMPA Letter. o Sponsored Residents & Clinical Fellows - are required to pay their own membership fees in full directly to the CMPA. There is no reimbursement from the WRHA. o CMPA is an organization that provides legal defense and liability protection to all physicians across Canada. Detailed information, including a downloadable form, please visit the CMPA website @ http://www.cmpa-acpm.ca/. D. College of Physicians and Surgeons of Manitoba (CPSM): o All postgraduate medical trainees must be registered with the CPSM prior to commencing the residency training. The CPSM Office will forward the registration application package directly to the trainees. o It is the trainee's responsibility to complete and return the application to the CPSM Office by May 16th, 2014. o o CPSM is required prior to the trainee's training start date. Failure to complete/submit the CPSM application by the posted deadline may result in a delayed start to the training. o The CPSM is the body that regulates the practice of medicine in the Province of Manitoba. The CPSM issues the certificate of registration authorizing postgraduate medical education on an annual basis. Letter of Offer: o The PGME Office will send the trainees the Letter of Offer via email prior to receiving the registration information. The trainee must review, sign and return the original Letter of Offer to the PGME Office. o The Letter of Offer is the confirmation of the trainee's acceptance of the offer on the terms and conditions noted in the offer. Once the Letter of Offer is received, the PGME Office will send the Registration Information to the trainees. F. Immunization: o It is the trainee's responsibility to complete the WRHA Immunization and Communicable Disease Record Form and Confidential Personal, Work and Health Questionnaire. o WRHA PGME Immunization Pre-Employment Letter o WRHA Immunization and Communicable Disease Record Form. o WRHA PGME Confidential Health Questionnaire (on the PGME Resources website – under letter “W” on the right hand side column) o The postgraduate trainees must contact one of the Occupational Health Units by May 16th, 2014 to schedule an appointment to attend prior to the commencement of training. Criminal Record Check (including Vulnerable Sector Verification): o It is a requirement that all postgraduate trainees must submit and be cleared on a formal Criminal Records Check. The Vulnerable Sector Verification must be included. Please check with the local law enforcement agency or from the Police Service that the Vulnerable Sector Verification is included in the Criminal Record. o Two original Criminal Record Check (including Vulnerable Sector Verification) are required One Month prior to the training start date: one to be sent to the PGME Office the other to be sent to the CPSM. o Post PHD - are required to submit the Criminal Record Check (including Vulnerable Sector Verification) to the PGME Office Only and NOT to the CPSM. Medical Diploma: o It is a requirement that all postgraduate medical trainees (with the exception of Post PHD) must submit a copy of a Medical Diploma to the PGME Office One Month prior to the training start date. o If the Medical Diploma is not in English, a notarized copy translated into English must be provided to the PGME Office One Month prior to the training start date. K. Social Insurance Number (SIN): o When completing Registration Information in VENTIS (a Postgraduate Medical Education Curriculum Management System), all postgraduate trainees will be required to provide their Social Insurance Number (SIN). o Non Canadian and Non-Permanent Resident - are required to apply for a Social Insurance Number (SIN). A copy of the Social Insurance Number must be provided to the PGME Office One Month prior to the training start date. Regional Health Authorities Learner Placement Affiliation Agreement: o The Agreement must be printed off, completed, signed and submitted to the PGME Office One Month prior to the training start date. o Regional Health Authorities Learner Placement Affiliation Agreement. M. New PGME Trainee Orientation: o All NEW postgraduate trainees are expected to attend the MANDATORY New PGME Trainee Orientation on Thursday, June 26th, 2014 at 8:15am to 4:00pm in the Frederic Gaspard Theatre. University of Manitoba Registration Fees: o WRHA Residents - the assessed University of Manitoba Registration Fees will be paid on the WRHA Resident's behalf by the WRHA via direct billing authorization through the Resident Employment Administration Office. o Clinical Fellows - the assessed University of Manitoba Registration Fees must be paid in accordance with the training agreement with their program. Lab Coats, Access Card, Identification and Pager: Once your CPSM enrollment status and CMPA membership status have been confirmed: o o o Lab Coats: Postgraduate residents will receive four new lab coats. The Authorized Requisition Form may be picked up at the PGME Office anytime after June 16th. Then the postgraduate residents must walk the requisition form over to the Sewing Room located in the basement level of the Laundry Building at the Health Sciences Centre. Clinical Fellows - NO lab coat will be provided. Post PHD - Lab coat will provided as per request only. WRHA/HSC Facility ID Access Card: Postgraduate trainees will receive a WRHA/HSC Facility ID Access Card. The Authorized Application Form may be picked up at the PGME Office anytime after June 16th. Then the postgraduate trainees must walk the application form over to the Security Services (between the hours of 9:00am - 3:00pm) located in Room MS245 MS Building (2nd Floor) at the Health Sciences Centre. This photo ID Access Card will be activated for the first day of training. Pager: All new postgraduate residents will be provided with a pager. University of Manitoba graduates accepted for the residency training program will carry their existing pager to the completion of training and are NOT required to get a new pager. Clinical Fellows & Post PHD - Pager will be provided per request only. Back to Table of Contents 15 o Following admission and registration of all new postgraduate medical trainees to the University of Manitoba, the PGME Office will communicate with all new trainees to confirm when the following accesses will be available. Further information will be provided. Aurora Student Employee Number the University of Manitoba Student Card (Photo Identification Card) Student Number and PIN P. Benefits: o The WRHA Resident Employment Benefits package will be provided prior to the commencement of training. Q. Citizenship and Immigration of Canada: o All postgraduate trainees who are NOT a Canadian or a Permanent Resident are required to have a valid Work Permit. It is the trainee`s responsibility to maintain a valid Work Permit and provide the PGME Office with a copy of the valid Work Permit(s). o Further information, please visit the website @ www.cic.gc.ca/english/work/applyhow.asp Back to Table of Contents 16 Trainee Access Once Registered Aurora Student: Aurora Student is our student registration system that also houses each student’s personal information. It is essential for all students to ensure that their personal information is correct and is kept up to date. o Activate your Email and UMnetID: The University of Manitoba will send important information to students, only using their U of M email account. o In order to receive important communications from the university, it is essential that you activate and maintain a university email account. For more details about your myumanitoba email account go to: umanitoba.ca/studentemail To activate a University of Manitoba email account go to: How to Activate your Email and Accounts and Claim your UMnetID. Maintaining your Personal Information: You may make changes to your personal information by accessing Aurora Student and then selecting Personal Information. o All mail will be directed to your current address provided. You may provide more than one address, as long as the dates of effectiveness do not overlap. Your primary phone number is the phone number associated with your current address. Additional phone numbers may be provided. Eg. Cell phone. To update your address or phone number, log in to Aurora Student. Go to Personal Information and select Update Addresses and Phones Back to Table of Contents 17 Change of Name: If you have changed your name since you first registered, official evidence of the name change must be submitted to the Registrar's Office with the Change of Name form. o The University of Manitoba uses your full legal name on its records, transcripts, and on graduation documents. Abbreviated or anglicized names should not be used unless they have been legalized with documentation. Employee Number: Once WRHA has received all your employment paperwork, you will be assigned an employee number. If you are a returning student, you will continue to use the student number assigned to you at admission. Your student number is recorded on your Student ID card. o o Your Personal Identification Number (PIN): New Students: When you log in to Aurora Student, you will be asked for your PIN — your Personal Identification Number. When you first access Aurora Student, your temporary PIN is your date of birth. Enter your date of birth numerically as YYMMDD (year, month, day). For example, if your birthday is February 8, 1985, you enter 850208 as your first PIN. Back to Table of Contents 18 You will immediately be required to change your PIN to another sequence of numbers for your security. This new PIN cannot be your birth date and it must be 6 to 10 characters long. Your PIN MUST contain numbers and MAY contain letters and symbols. Letters used WILL BE case sensitive. Choose a PIN that cannot be easily guessed by others, and remember to memorize your PIN. (see PIN Security Question below). Returning Students: If you are a returning student, use the same PIN that you used the last time you logged into Aurora Student. If you have difficulty with your PIN, call the Registrar’s Office at (204) 474 9420 for help. All Students are Responsible for your PIN. Keep your PIN confidential. You are responsible for all activity that takes place with the use of your Student Number and PIN. You can change your PIN again, and as often as you wish, through Aurora Student. Go to Personal Information and select Change PIN. VENTIS: VENTIS is a web-based Postgraduate Medical Education Curriculum Management System which is used to manage postgraduate registration, scheduling (rotational schedule, daily assignments, call schedules and educational events) and assessment. o Trainees will be provided access to VENTIS prior to commencement of training. Back to Table of Contents 19 Resident Employment Winnipeg Regional Health Authority (WRHA) You will have, and maintain, an appropriate medical staff appointment with the WRHA in accordance with the WRHA’s Medical Staff By-Law, which requires that you comply with the policies and procedures of the WRHA. THE WRHA Personal Health Information Act (PHIA) pledge of confidentiality information and detailed will be provided at the MANDATORY New PGME Trainee Orientation on Thursday, June 26th, 2014. Trainees with the late commencement of training – will be provided information about online PHIA Session/Information from the PGME Office. 20 Professional Association of Residents and Interns of Manitoba (PARIM) PARIM is a volunteer, non-profit organization representing physicians in training; PARIM strives to achieve excellence in medical education by: 1. A: We do not retrieve documentation from UGME from past electives Q: Can I send my registration documents electronically by email A: The preference is original by mail or courier. Q: I have provided CPSM with my criminal record check and medical diploma can you obtain this information from them? A: We do not solicit documentation from the CPSM on behalf of the resident for our registration process. Q: What address do I put on my registration form? A: If you do not have a Winnipeg address please indicate your home address and provide the PGME office with your local address once established. Canadian Medical Protective Association (CMPA) Q: I'm trying to fill out my CMPA application online. At one point it asks for a License/Registration Number and Type of License - at present I have indicated that I do not know either of these. A: In Manitoba, The CPSM educational license does not have a number assigned to it Q: Toward the end of the application, it asks whether I will "be taking a specialty certification exam at the end of your program" - I'm unclear whether this question is referring to the Royal College exam all residents take A: RCPSC and CFPC residents will sit a specialty/subspecialty exam at the end of training. 24 College of Physicians and Surgeons of Manitoba (CPSM) Q: I have provided CPSM with my criminal record check and medical diploma can you obtain this information from them? A: We don’t solicit documentation from the CPSM on behalf of the resident for our registration process. A: The PGME office does not share or provide resident/fellow registration documentation to CPSM Immunization Requirements Q: I have submitted my Immunization information do I still require an Appointment with OESH? A: The WRHA OESH health record/immunization appointment needs to be made by May 16, 2014. Criminal Record Check (including Vulnerable Sector Verification), Child Abuse and Adult Abuse Registry Check Q: I am having some difficulties trying to determine what is required for the police background check A: All trainees are required to complete a Criminal Record Check (including the Vulnerable Section Verification), a Manitoba Child Abuse Registry Check and Adult Abuse Registry Check. Any police or RCMP station can assist you with a criminal record check (which usually includes the Vulnerable Sector Verification). The Child Abuse Registry and Adult Abuse Registry Check are a separate process. You may contact their office (contact information indicated above under each section) for detailed information and process. Q: Can I obtain a Child Abuse Registry Check from another province? A: No, we require Child Abuse Registry Check to be for Manitoba Q: Is my police report with Vulnerable Sectors enough or is the Child/Adult Abuse Registry Check separate? A: The Criminal Record Check will usually include a Vulnerable Sector Search, but is not a Child/Adult Abuse Registry Check. This is a separate process, and you will have to contact their office (contact information indicated above under each section) for detailed information and process. 25 Health Insurance Coverage Q: I do not have a Manitoba Health number A: Please indicate your current provincial health number and provide an update once you've arranged for Manitoba coverage. There is a 3 month waiting period for resident coming from another province. Q: The registration documents indicate that we must have health insurance coverage A: This refers to your provincial health care coverage. If you live in Manitoba, then you will have a provincial MB health number. If you are coming in from another province, you would carry coverage from your home province, but will have to apply for Manitoba coverage once you arrive. You may provide your current provincial health number on the application form and provide our office with an update University of Manitoba Student Number Q: How do I obtain my University of Manitoba Student Number? A: The U of MB student number will be available in Aurora Student and updated in VENTIS system once you have been admitted by the University of Manitoba and the PGME Office Letter of Offer Q: What specifically is the Signed Letter of Offer? A: The letter of offer is your formal acceptance to residency training with the University of Manitoba and employment as a resident with the Winnipeg Regional Health authority. It is is signed by Dr B Wright and Dr C Yaffe and is a first point of communication to you from the PGME Office. 26 Personal Insurance Q: Does Blue Cross provide a plan for residents? A: Blue Cross is the personal insurance that will be offered through your registration with the WRHA, plus there are other personal insurance packages that will also be offered through Doctors Manitoba. Both the WRHA and Doctors Manitoba will be presenting their information at the orientation on June 26th, 2014 and providing you with coverage options to choose from. There will be a deadline for enrollment, and this will be further highlighted in their information presentation/package. Medical Diploma Q: Do I need to provide you with a copy of my degree if I graduated at U of Manitoba? A: Yes, you are required to provide a copy of your medical diploma Q: My medical diploma is very large, can I submit a smaller version? A: Yes, please provide a smaller 81/2 x 11” size copy.
