SFSP Application Packet Due Dates
Applications open March 1- it is in your best interest to submit the SFSP application packet prior to the due date to allow any
needed corrections to be made prior to, or by, the due date. The due dates outlined below indicate the last day a complete
application packet is due to the CDE OSN.
• April 15- if the sponsor is requesting an advance payment
• April 15- all new non-School Food Authority (SFA) sponsor applicants
• May 1- all renewing sponsors
Note: Once the sponsor is approved, additional site applications can be added through June 30.

Common Application Questions
1.
What If I Don’t Remember My Username or Password?
Contact Stacey Macklin at: [email protected]. Your username will be provided and/or your password reset. Please
remember to write your username and password down. Note: usernames and passwords cannot be shared. If additional sponsor
personnel require a username and password, contact Ashley Moen at: [email protected].

Where Are The Site Applications?
The site applications are found by clicking on ‘Summer Food Service Program’ below the gray bar that states Site Applications.

How Do I Determine Site Eligibility?
Follow the instructions on the How to Determine Site Eligibility document. The CDE October 1 count pK-12 data is the data used to
determine site eligibility. If another month’s free and reduced-price lunch percentage is used, that month must be indicated in the
site application in #26 next to the school district name. Note: the school data used must be for the school in the attendance area of
the site.

What Do I Select For The Site’s Meal Service Method?
If the sponsor contracts with a vendor to prepare and deliver meals, the meal service method will be ‘purchased from a vendor’. If
the sponsor contracts with a School Food Authority (SFA) to prepare meals for the site, the meal service method will be ‘purchased
from a SFA’. If meals are prepared at the site or the sponsor’s central kitchen, the meal service method will be ‘prepared on site’.

How Do I Determine A Site’s Average Daily Participation (ADP)?
The ADP should be calculated based on last year’s attendance, anticipated enrollment in the site activity, or the estimated number
of children within walking distance to the site. The ADP entered in the site application must be as accurate as possible. If the site’s
ADP is much lower than stated in the application, the ADP must be revised in order to update the sponsor budget. If the site’s
ADP is higher than stated in the application, the ADP must be revised prior to entering a claim.

How Do I Submit A Field Trip Request?
The field trip request form is located in the online application packet under ‘Site Field Trip List’. The field trip form must be
completed for all field trips that will take place at the site. If the site has a recurring field trip, only one form needs to be completed
and multiple dates can be entered. If the site has field trips to multiple destinations, separate field trip forms must be completed.
7.
Do I Complete The Food Production Facility List?
The food production facility list section of the application packet is only completed for sponsors that are contracting with a vendor
or SFA, or have a central kitchen where meals are being prepared for the site. If all meals are prepared at the site(s), the food
production facility list does not need to be completed.

Please reference the application instructions as you are completing the application packet.

Documents that are required to be submitted with the application packet include:
o Vendor Packet (W-9): Required for new sponsors
o Permanent Agreement: Required for new sponsors
o Documentation of Tax Exempt Status: Required for new private non-profit sponsors (not required for churches)
o Proposed Media/News Release: Required for all site types
o Health Department Notification Letter: Not required for sites that receive inspections through the NSLP/SBP or CACFP
o SFSP Invitation For Bid: Required for sponsors contracting with a vendor above the small purchase threshold
o Vended Meal Contract Alternate Form: Required for sponsors contracting with a SFA or a vendor below the small
purchase threshold

The application packet must reflect current practices and the application packet must be updated with any changes. Sponsors
must request the following site changes at least one day before the change is to take effect:
o Meal types offered
o Meal service start and end dates
o Meal service start and end times
o Field trip requests

Make sure that the site contact information is accurate. The site address, phone number, and operational information are
submitted to the kids food finder map so children and families are able to access summer meals.
•
Don’t forget that the application packet is not submitted until you click the red ‘submit for approval’ button.
