Outline A ten year review assessing clinical outcomes (mortality, blood loss, ICU and hospital length of stay etc) in all patients who had high spinal anesthesia with local anesthetics for their cardiac surgical procedure, and comparing them to a controlled, matched cohort of patients who did not have high spinal anesthesia 1. Arora (Cardiac Surgery), Dr. H. Grocott, Dr.D. Maguire, Dr. A. Petropolis (Anesthesia resident). A review focusing on patients who have had cardiac surgery since 2010 to the present. Since 2010, postoperative cardiac surgical patients have been formally assessed for the presence or absence of delirium by ICU nurses using the Confusion Assessment Methodology (CAM) protocol. We will compare the incidence of delirium in patients who have had high spinal anesthesia for their cardiac surgical procedure with a matched cohort of patients who did not have high spinal anesthesia. Maguire, Dr. Cenkowski (anesthesia resident) Patients will be randomized to receive either low dose bupivacaine (4.5 mg) or conventional dose bupivacaine (9 mg) for their spinal anesthetic. Patients will have an arterial line for the procedure and have their cardiac outputs measured using arterial waveform analysis (Flo Trac). Hemodynamics will be compared between the two groups as well as secondary outcomes such as pain scores, patient satisfaction scores and recovery room length of stay. Traumatic Brain Injury Study National, multi-center observational Dr. Alexis Turgeon (principle) Dr. S. Kowalski (site coordinator) Patients with traumatic brain injury and a Glasgow Coma Score of 8 or less will be assessed using repeat clinical examinations, serum biomarkers for brain injury, CT scans, MRI scans, EEG and sensory evoked potentials for ten days post injury. Repeated follow up will be done at six months and twelve months post injury for functional neurological recovery using the Glasgow Outcome Score. Correlations of prognostic tests, examinations, clinical data and their evolution over time will be made with long term outcomes in an attempt to develop a reliable, valid and clinically useful prognostic model for patients with traumatic brain injury. Mykytiuk A ten year review of major complications of regional anesthesia (specifically epidural hematoma and epidural abcess) and associated risk factors that have occurred at the Health Sciences Centre. The effect of intraoperative cerebral oxygen desaturation on postoperative delirium in thoracic surgical patients Prospective, observational Dr. S. Kowalski, Dr. S. Srinathen (thoracic surgery), Dr. P. St. John (internal medicine), Dr. H. Grocott, Dr. W. A. C. Mutch, Dr. R. Amadeo, Dr. T. Mutter, Ms. L. Girling (BSc Hons.) To determine the current incidence and risk factors for post-operative delirium in thoracic surgical patients. All patients will be monitored intraoperatively using near infrared spectroscopy (NIRS) for cerebral desaturation to determine whether cerebral desaturation is associated with post-operative delirium. Weaning from Prolonged Mechanical Ventilation: Use of Speaking Valve Trials Retrospective, observational Dr. S. Kowalski, Dr. Z. Bshouty, Dr. A. Robertson, Ms. K. Macaulay RRT, Ms R. Thorkelsson RN BN, Ms. L. Girling (BSc Hons.) A review of the use of speaking valve trials in weaning patients who have required prolonged mechanical ventilation. Thorkelsson RN BN, Ms. L. Girling (BSc Hons.) Patients who are having speaking valve trials as part of their weaning program will be studied. Cough strength will be assessed using a peak expiratory flow meter under two conditions Patients will be asked to cough through the tracheostomy tube, with the cuff inflated and then around the tracheostomy tube with the cuff deflated and a one-way valve attached to the tracheostomy tube. A Review of Decortications in Manitoba from 2007 - 2014 Retrospective, observational Dr. S. Kowalski, Dr. S. Srinathen (thoracic surgery), Dr. J. Embil (Infectious Diseases), Dr. Duane Funk (Anesthesia), Mr. Jay Gorman BSc, Ms. L. Girling (BSc Hons.) There has been a greater than fivefold increase in the number of decortications in Manitoba since 2007. The review will describe the characteristics and the anesthetic, surgical and anti-microbial management of these patients.
