UTB ID Number: 7-digit number assigned to the student by UTB. Student Name: Name, Middle Initial and Last Name of the student. Student Phone Number: Student contact phone number. Department: Name of employing department. Division: ACAD (Academic Affairs), BAD (Business Affairs), CE (Continuing Education), EDCS (Economic Development & Community Services), IAD (Institutional Advancement), ITS (Information Technology Services), PAD (Partnership Affairs), PRES (President’s Office), PROV (Provost’s Office), RD (Research), SAD (Student Affairs). Supervisor: Name of the student employee’s immediate supervisor. Phone Number or Extension: Campus phone number of the immediate supervisor. Email: Campus email address of the immediate supervisor. Hiring Semester: Choose the semester you want the student to work from the drop down menu. (MAY, SS1, SS2, FALL, SPRING) Proposed Start Date: Enter the date you want the student to begin working for your department once your request is approved by the Office of Global Engagement and the Office of Human Resources. The start date must be on or after the first day of classes for the semester indicated on the request form. Proposed End Date: The end date must be on or before the last day of classes for the semester indicated on the request form. Job Title: Must use a job title from the UTB Pay Plan at http://www.utb.edu/ba/hr/payplan/. Number of Hours; The number of hours the student is expected to work per week. Students can work only in Spring & Fall semesters 20 hours per week. 40 hours are permited during official school breaks ( winter, summer) or for students that are on Optional Practical Training. Supervisor Signature: Signature of the immediate supervisor of the student employee. Date: Date the immediate supervisor signed the request form. Please route this request to the Office of Global Engagement (EDBC 1308), attention: Aragelia Salazar at least two weeks before you want the student to start working for your department. Once this form has been received at OGE with all the required documentation, we will process it within three (3) business days. Along witht this form, the hiring department must prepare the online Recomendation for Hiring Form (RHF) at https://ba.utb.edu/portal/ Section to be completed by the Student Employee Student must read and sign the disclaimer and also provide copies of the following documents to the Office of Global Engagement: I-94 arrival departure record (both sides). If student no longer has the old white card (I-94), they must attached their electronic I-94 number page. Copy of I-20 (F-1) or DS-2019 (J-1) Copy of valid passport identity page Copy of student visa, students on Optional Practical Training must provide their Employment Authorization Document (EAD) Provide receipt of Social Security Administration ***Students with OPT authorized may work on campus full time. EAD will determine the employment eligibility dates. Section to be completed by the Office of Global Engagement The Office of Global Engagement will complete this section of the form. A student advisor from the Office of Global Engagement will verify that the student is in good academic standing and is authorized to work in the United States before routing the form to the Office of Human Resources for final approval. The Office of Global Engagement and Office of Human Resources must approve (a signature is required) this request before the student can begin working for UTB. Once student has been authorized by OGE and HR, he/she must complete the new hire packet at Human Resources at http://www.utb.edu/ba/hr/employment/Pages/newemployees.aspx.When student receives original social security card, he/she must provide this document to the Office Human Resources for clearance on E-verify. I am also aware that this authorization to work is valid for one semester only, unless student is on OPT. Thus, if I wish to rehire this student employee next semester, a new request must be completed by the hiring department and submitted to the Office of Global Engagement for approval before the employee can continue to work for UTB. I am also required to check the Payroll Paperwork Deadlines at:http://www.utb.edu/ba/budget/Documents/payroll-deadlines.pdf to make sure the student employee receives compensation in a timely matter.*Students on Optional Practical Training can work from 20-40 hrs. per week.
