Community Resources
Who can participate in Breakthrough ?
Breakthrough helps students make it
through school safely and successfully.
The Breakthrough Student Assistance
Program is a unique method for providing
family specific services to any family in
the school district. Typically, this type of
service is only available in most school
districts if the student is thought to be in
need of a Student Study Team, or the student has an identified disability (IEP or
Section 504). Breakthrough is available
to any family.
Support Groups:
Drug/Alcohol Prevention (Insight)
Anger Management
Poor Attendance/Achievement
Grief
Stress
Students are referred by staff, parents, or concerned others to the Breakthrough Program.
Students may also self-refer or may be directed
to participate in one of the program components
in response to a violation of the school behavior
policy.
A Family Conference is scheduled:
The family participates in a motivational interview with a Breakthrough staff member at the
Breakthrough Office conveniently located at the
District Support Center. The family and Breakthrough staff member work collaboratively to
develop a Family Service Plan designed to help
the student be safe and successful at school. Appointments may be scheduled by using our
online scheduling program. Visit our website:

The Family Conference will assist in several
ways:






Identify family strengths and how to utilize
them.
Explore the student’s academic and career
goals as well as social and emotional needs.
Identify major concerns of family.
Create plan of action to foster success for all
family members.
Breakthrough staff will provide ongoing support as needed.
Family will obtain knowledge of resources
within the community.
