A factor contributing to the accident was inadequate maintenance procedures to detect fatigue cracking.“ • FAA issues AD 2002-19-14 on Sep 26, 2002 – Mandating wing inspections on all C-130A aircraft. Lockheed • C-130B/H and L-382 Center Wing actions: – Prior to June 2002 Lockheed was supporting USAF with Center Wing fatigue cracking issues. – 2002 HOC presentation: Center Wing Durability Issues the first in a series of Center Wing presentations continuing through 2007 HOC. – 2004 Lockheed begins major analysis effort for Center Wing life. – 2006 Lockheed completes the Center Wing service life analysis. Lockheed • 2005 Lockheed issues two Service Bulletins for L-382 aircraft: – SB 382-57-84 - Operational Usage Evaluation – SB 382-57-85 - Center Wing Lower Surface Generalized Cracking and Widespread Fatigue Damage Inspection Requirements BL 61 Cracking Related FAA Actions • Apr 18, 2006 - Notice of Proposed Rulemaking (NPRM) issued for Aging Aircraft Program – Widespread Fatigue Damage (WFD) • Nov 10, 2010 - Final Rule issued for Aging Aircraft Program – WFD / Limit of Validity. • May 18, 2011 – AD 2011-09-04 issued, mandating accomplishment of SB 382-57-85. • Jun 14, 2013 – Limit of Validity (LOV) limits in effect – 75K hours or 58.8K cycles. – Center Wing Life Limit AD to support LOV due ??? LAC Actions • Accomplished SB 382-57-85 as aircraft cycled through C Checks. • Based on risk evaluation from the Widespread Fatigue Damage (WFD) proposed rule, now the LOV final rule – issued Nov 15, 2010 - and proposed Center Wing life limits, 3 each replacement Center Wings are required to support a five aircraft fleet for LAC L-382G operations beyond Q213. LAC Actions • Decision – to procure and replace Center Wings on N401LC, N403LC and N406LC. • Began discussions with Lockheed in 2007. • Contract signed Sep 10, 2008. • Center Wing No. 1 delivered Dec 16, 2010. – Flown to Singapore onboard LAC Herc. – N403LC check input STAE on Sep 11, 2011. – Center Wing installation complete Jan 7, 2012 (118 day span) including C10 check and full paint. 1st L-382 Center Wing Replacement One done, two to go by STA Engineering and Lynden Air Cargo 7 January 2012 One done, two to go The End • Show video
