The Financial Aid & Scholarships Office has received a Federal TEACH Grant application for the above named student. Through the
College Cost Reduction and Access Act of 2007, Congress created the Teacher Education Assistance for College and Higher Education
(TEACH) Grant Program that provides grants of up to $4,000 per year to students who intend to serve as full-time teachers in a highneed field, in schools serving low-income students for a minimum of 4 years.
In order for our office to continue to process the student’s Federal TEACH Grant application we must verify that the student is
enrolled in approved coursework that will allow him/her to teach in one of the high-need fields or in one of the listed Nevada
Shortage Areas. Please complete the information below and return this form to the student to submit to the Financial Aid &
Scholarships Office.
