MINISTER'S SCHEDULE OF REQUIRED FEES FOR SERVICE PROVIDERS made under section 1 2 1 . 1 of the
INSURANCE ACT
R.S.O. 1990, c.I.8
INTERPRETATION
Definitions
1. In this Schedule,
"claimant" means a person in respect of whom an applicant or licensed service p~ovider, as applicable,
received payment for one or more listed expenses, in the calendar year prior to the calendar year in
which payment of the applicant's or licensed service provider's annual regulatory fee is required.
"fiscal year" means April 1st to March 31st.
"initial serviee provider's licence" means a service provider's licence to be issued effective December 1,
2014.
"listed expenses" means listed expenses in connection with statutory accident benefits within the
meaning of section 288.1 of the Insurance Act notwithstanding its coming into force date.
"location" means, in respect of an applicant or licensed service provider as applicable, a physical
location at which the applicant or licensed service provider carried on business that gave rise to listed
expenses, in the calendar year prior to the calendar year in which payment of the applicant's or licensed
service provider's annual regulatory fee is required.
"new service provider's licence" means a service provider's licence issued to an applicant who was not a
licensed service provider in the fiscal year prior to the year in which their licence is issued but does not
include an initial service provider's licence.
"Superintendent" means the Superintendent of Financial Services appointed under the Financial Services
Commission ofOntario Act, 1997, S.O. 1997, c.28.
LICENCE APPLICATION FEE
Service provider licence application fee
2. A person or entity who applies for a service provider's licence shall pay a licence application fee
of $337.00 when the licence application is submitted to the Superintendent.

ANNUAL REGULATORY FEE Service provider annual regulatory fee
Initial applicants' regulatory fee
3. (1) A person or entity who applies for an initial service provider licence shall pay a pro-rated
regulatory fee when the licence application is submitted to the Superintendent, calculated using the
formula,
(A+ B) X (4/12)
where "A" is $128.00 multiplied by the applicant's number oflocations and "B" is $15.00 multiplied by the
applicant's number of claimants.
New licensees' regulatory fee
(2) A person or entity who applies for a new service provider's licence shall pay a pro-rated regulatory
fee when the licence application is submitted to the Superintendent, calculated using the formula,
(A+ B) X (X/12)
where "A" is $128.00 multiplied by the number of the licensed service provider's locations, "B" is $15.00
multiplied by the number of the licensed service provider's claimants, and "X" is the number of full and
partial months remaining in the fiscal year, calculated from the date application is made until March 31st.
Licensees' annual regulatory fee
(3) A licensed service provider who holds a service provider's licence that is not a new service provider's
licence, shall pay a regulatory fee annually when the service provider's annual information return is
submitted to the Superintendent, calculated using the formula,
(A+ B) X 12
where "A" is $128.00 multiplied by the number of the licensed service provider's locations, "B" is $15.00
multiplied by the number of the licensed service provider's claimants.
Commencement
4. This fee schedule comes into force on June 1, 2014.
