Dr. Suchak
Dr. Suchak serves as a geriatrics consultant
providing health assessment and management recommendations for older adult patients. Her goal is to make FSU SeniorHealth
at Perry a partner with the patient and primary care physician in optimizing the patient’s health and wellness.

‘Expert Consultation Services
for Older Adults’
Her Role Includes
Conducting initial and periodic comprehensive evaluations;
Developing an integrated care plan;
Recognizing, investigating and making
treatment recommendations for geriatric
syndromes;
Reviewing and recommending preventive
care interventions;
Improving adherence to lifestyle changes;
Improving patient self-management skills;
and,
Providing patient, family, and caregiver
education.
